Community Nursing Care

Specialised care by registered nurses for participants with complex health needs, including medication management, wound care, catheter care, diabetes support, and chronic condition monitoring.

Home Modification

Physical changes to a participant’s home—like installing ramps, grab rails, or accessible bathrooms—to improve safety, accessibility, and mobility within the home environment.

Supported Independent Living (SIL)

Ongoing daily support in shared or individual living arrangements to help participants with personal care, cooking, cleaning, and skill development for independent living.

Daily Living and Life Skills

Training and support to develop essential life skills such as cooking, budgeting, personal hygiene, communication, time management, and travel training, empowering participants to live more independently and confidently in their homes and communities.
